Insurance
Ninety Six Family Dentistry gladly accepts most insurance providers. Please bring your updated insurance information with you to your next appointment so that our staff can expedite reimbursement. Our patients are responsible for all fees charged by Ninety Six Family Dentistry regardless of insurance coverage. Ninety Six Family Dentistry Membership Program
Lifetime Activation Fee covers your first month.
Membership fees may be prorated for new members added to an existing account.
Adult Membership

Professional dental cleanings (2/year)
Doctor exams (1-2/year)

For patients who receive regular cleanings and who do not have periodontal disease. Ages 13 and older. $99 Lifetime activation.
Perio Membership
$43/mo.
Perio maintenance cleanings (3-4/year)

For patients enrolled in active periodontal maintenance due to prior treatment for periodontal (gum) disease. $99 Lifetime activation.

Legal & Disclaimers
Membership clubs are not insurance but rather payment arrangements provided by Ninety Six Family Dentistry for services rendered. Memberships are provided exclusively to uninsured patients of our practice and shall not be considered pre-payment for future services or payment for access to discounted services. Rather, our patient members are electing to make regular monthly subscription payments for preventative services instead of paying at the time of each service. Payment for any cosmetic or restorative treatment is due at the time of service. Membership dues must be current to receive services. A lapse of 28 days in dues will require a reactivation fee of six months’ dues. Membership club discounts may not be combined with any other offers or discounts. It is solely the patient’s responsibility to schedule and keep their appointments. Under no circumstances will refunds be provided for dues paid, including failure to schedule and maintain appointments. A patient member’s monthly agreement is with the legal entity of Ninety Six Family Dentistry. Monthly dues and fees for dental services may change at any time.
